As our patient; you have important rights relating to inspecting and copying your medical information that Capeside maintains, amending or correcting that information by obtaining a disclosure of accounting, by requesting Capeside communicate with you confidentially, and as the patient the right to request certain restrictions of disclosure related to your health information, and filing a complaint with Capeside Privacy Officer when you believe your rights may have been violated.

Capeside has a detailed Notice of Privacy Practices which fully explains your rights and Capeside’s obligations under the Health Insurance Portability Accountability Act (HIPAA). Capeside may revise the Notice from time-to-time. The Effective Date at the bottom of the page indicates the most current Notice in effect.
